Community Services

The SHARE Network is older adults, caregivers, providers, and aging specialists working together, sharing knowledge and resources, to bridge the gap between geriatric specialty care and the Chicago South Side community.

A Memory Cafés is a space for seniors with memory loss and their care partners to have fun, eat snacks, socialize and learn about memory related health and wellness resources. The group meets the 2nd Tuesday of each month from 10 -11:30am.

Updates and information can be found @LaBrochaChicago on Facebook Provides free art workshops to Latinas/os 60 years and older, including those with memory loss, Alzheimer’s or Parkinson’s disease. The workshops are led by a teaching artist who guides all participants step-by-step through a featured painting. NO art experience is required.

Provides a several programs (caregiver support, care coordination, and adult protective services among others) for older adults that enhance independent living.

The PACE program in Illinois coordinates and provides care and services to older enrollees with chronic care needs to enable them to continue living safely in the community as long as possible.

Works to empower Latino caregivers and families of those afflicted with Alzheimer’s disease and memory disorders by providing education, skill building training and support programs, and training programs to healthcare providers.

Offers screenings and counseling so seniors and provides their families the opportunity to ask questions, discuss needs, and understand options for continuing care.

Provides comprehensive services so seniors are able to continue to live independently or with caregivers in their own homes and communities.

Government program that assists low income older adults who need help with activities of daily living by helping to pay for home and community based services including: care management, in-home care, adult day care, transportation and home delivered meals.

Connects residents with vital services such as information and assistance; community dining programs and home-delivered meals; housekeeping help; employment services; access to benefits; and provides support for family caregivers.
